Within the framework of the General Privacy Notice under the Personal Data Protection Law and the MT Studio Regulation on the Protection and Processing of Personal Data, provided by BOMONTİ MUAY THAI SPOR EĞİTİM DANIŞMANLIK VE ORGANİZASYON TİCARET LİMİTED ŞİRKETİ (hereinafter “MT Studio”) in accordance with the relevant provisions of Personal Data Protection Law No. 6698 (“the Law”):

I acknowledge and declare that I have been informed by MT Studio and give my explicit consent under the Law to the processing of my identity, address, contact and other personal data directly or indirectly related to the following purposes: enabling MT Studio to provide its products and services under the best conditions; ensuring reliable and uninterrupted supply; maximising customer satisfaction; making payments; performing transactions related to those services; conducting and improving operations; promoting, marketing and advertising those or other products and services and conducting campaigns; informing customers about offers, campaigns and other services; and fulfilling contracts concluded with customers.

Such processing may be performed by MT Studio as data controller or by processors it authorises subject to the necessary security measures. My data may be obtained, recorded, securely stored in physical or electronic form for a period appropriate to the processing purpose, retained, modified, reorganised, disclosed and transferred in accordance with the law, acquired, classified, processed or restricted from use. Processing shall comply with the general principles in Article 4 of the Law, particularly retention only for the period prescribed by legislation or necessary for the purpose of processing.

I acknowledge and declare my explicit consent to MT Studio transferring my personal data, solely for the purposes stated above, to its employees, officers, group companies (MT Studio and/or its business partners and shareholders), legally authorised public institutions and organisations, independent auditing companies and survey companies within legal obligations and restrictions for the conduct of its activities, and business partners and service providers from which MT Studio obtains services or with which it works to provide services and/or conduct activities for me.

I also acknowledge that, under Article 11 of the Law and relevant legislation, I may apply to MT Studio to:
• Learn whether my personal data is processed.
• Request information if my personal data has been processed.
• Learn the purpose of processing and whether the data is used in accordance with that purpose.
• Learn the third parties to whom my data is transferred in Turkey or abroad.
• Request correction of incomplete or inaccurate processing.
• Request deletion or destruction when the reasons requiring processing no longer apply.
• Request notification of correction and deletion requests to third parties receiving the data.
• Object to an adverse result arising from analysis of my data exclusively through automated systems.
• Request compensation for damage caused by unlawful processing.

To exercise these rights, I may complete the form at http://www.muaythaistudio.com, including the information necessary to identify me and an explanation of the right I wish to exercise, and deliver a signed copy personally with identification documents to MT Studio at Cumhuriyet Mahallesi Nehir Sokak Gündüz İş Merkezi No: 3/2 Şişli / İstanbul, send it through a notary, or use another method specified in the Law.

I further acknowledge and declare that the personal data I have shared with MT Studio is accurate and up to date, and that I will notify MT Studio of any changes.

I acknowledge, declare and undertake that I explicitly consent to the processing of my relevant personal data, including special categories of personal data defined in the Law; its use and sharing limited to the purpose of processing in the relevant process; and its retention for the required period. I confirm that I have received the necessary information and have read and understood this text, the MT Studio Regulation on the Protection and Processing of Personal Data, and the Privacy Notice.
